High School Combine 2023

I hereby give my consent for the above named student to participate in the Prep Ball Stars/Sports Ventures International event. I understand Prep Ball Stars/Sports Ventures International cannot accept responsibility for personal items lost or stolen. I authorize Coach, Trainer or Sponsor in attendance at Prep Ball Stars event to select and secure medical attention as may be necessary for my child as a result of injuries or other events requiring emergency care while I/we are not in attendance at such event. I hereby release said official or Sports Ventures International from any and all liability on account of such selection or authorization for any and all damages which occur on account thereof. Liability Release: I hereby release Prep Ball Stars/Sports Ventures International and its owners, trainers, agents and employees from any and all liability in conjunction with participation and as a result of any and all activities relating to Prep Ball Stars events, including Covid 19. I understand the assumption of risk associated with basketball activities and acknowledge acceptance of these terms is done on a voluntary basis. The Right of Publicity: I consent to allow Sports Ventures International to use name and likeness (video and photos) for promotional use for Prep Ball Stars/Sports Ventures International related events.
